A week or two in, we go through how you are taking it and what you have noticed.

Follow up

Two more conversations check side effects and whether the medicine is doing its job.

Fewer surprises

Problems get caught early, so you are less likely to stop a medicine that could have worked.

How it works

Tell us it is a new medicine for a long-term condition and we will offer the service.

Talk to the pharmacist

The first conversation happens once you have been taking it for a week or two.

Two follow-ups

We check in again to see how you are getting on and whether anything needs changing.

Questions about this service

Who can use it?
Anyone starting a new medicine for a qualifying long-term condition, including asthma, COPD, type 2 diabetes, high blood pressure and some others.
Do I have to come in?
The conversations can happen in the pharmacy or by phone, whichever suits you.
Does it replace my GP appointment?
No. It sits alongside your GP care, and we tell your surgery if something needs their attention.
